YOU ARE RESPONSIBLE FOR YOUR TEMPORARY TATTOO LICENSE IF YOU ARE FROM OUT OF STATE. I have placed the forms and links, below this application. You must have this submitted and approved by the Oklahoma State Department of Health at least 30 days before the event, no exceptions.

License and Lawful Affidavit below.
You will need these, if you are coming from out of state.
Turn these in to the OSDH (Oklahoma State Department of Health) in a timely manor.
